Not necessarily. I can put an unaltered copy of the GPL in a file named
"GPL" and then at the top of every other file say:
"Copyright 2006 John Hasler. You can redistribute this program and/or
modify it under the terms of version 2 of the GNU General Public License
as published by the Free Software Foundation except for X and Y. A copy
of the GNU General Public License will be found in the file GPL."
