Allow :
 [WHEN SET TO] FALSE [IS] literal
and
SET identifier TO FALSE

for 88 level (2002 standard)


PROCEDURE/ENTRY USING identifier whereby
identifier is REDEFINES is not allowed.

Allow ACCEPT identifier FROM ENVIRONMENT literal
(ACU extenstion)

Fix SET xxx TO TRUE
when xxx is not 88 level ( oops seg fault)

Don't generate NULL params for !cb_sticky_linkage

Fix cb_int usage

Fix CONTINUE usage
