I just stuck one up at:
http://alumni.engin.umich.edu/~oberon/cvs1-11.html.tgz
and
http://alumni.engin.umich.edu/~oberon/cvs1-11.html.tar.bz2
If you have Tex installed, you can generate it from the distribution's
cvs.texinfo file yourself using the texi2html command.
