Item A is on reserve for Patron A.  Patron B find the item on the shelf and goes
to check it out.  Librarian sees that Item A is reserved for Patron A, but
overrides the reserve without cancelling the reserve.  The item is issued
correctly to Patron B, but the items is also made waiting for Patron A.  Thus
Patron A will see on their account that the item is waiting for them, but when
they go to the library they will find that the item has been checked out by
someone else.
