Applied both patches with minor whitespace fixup.

I suspect we were depending on c->dwarf.ip = 0 terminating the unwind
at some point and after this commit:

d04dc94 dwarf: ip == 0 should't terminate unwind

a few tests broke and I suspect this is one of the cases that didn't
get fixed up. It fixed a couple of test failures too. Thanks.
