Here is a definitive template to hymn using \partcombine,  
\partcombineApart, \partcombineChords, \partcombineAutomatic and \lyricsto.

There are 3 files (hymn.ly, common.ly and hidden_parts.ly)
The hymn.ly is the template, read to use.
The common.ly have common notation for hymns (lyric fonts, breathes,  
refrain text and others you can put)
And the hidden_parts.ly contain hidden voices to use with \lyricsto.

I created aliases for \partcombineApart (\u), \partcombineChords (\j) and  
\partcombineAutomatic (\auto) for easy code entry.
