The bug is reproducible with 2.15.2 (Jim used 2.16.1) and the output is
improved with 2.17.13, a more recent development release.
Report from Jim follows:
Please educate me if my rhythmic staff technique is to blame, but
the attached example shows the use of a fermata in a
RhythmicStaff context.
My measurements here are approximate, but ....
When combined with a zero-line staff and \stemDown, the fermata
is engraved about one staff space or so above the zero-line
staff, which seems understandable.
However, the notehead and tie extend more than a staff space
outside the staff, resulting in a collision with the fermata.
The second score uses a five-line staff with no stencil. This is
not a workaround, but just an example showing graphically what
IMO would be a better default behaviour in the original case of
the zero-line staff and \stemDown.
