Wow. Are any of her examples well-known pieces, so we can see how the engravers handled it? Tedd Ross agrees with Gould, and shows (image) how to handle the situation with an extra bar-line.
We can easily repair the order at start-of-line,but LilyPond is not configured to look at the /type/ of bar when figuring the order of "break-align" objects, nor to create an extra bar-line. For a while, those who want to follow Gould might need to work-around the mid-line cases with stupid grace tricks {b1 \time 2/2 \grace s4 \repeat volta 2 b1}
