My immediate gut reaction is that the OttavaBracket grobs in the above  
example, or any grob whose alignment depends on that of multiple vertical  
axis groups, needs to be marked cross staff.  In general, any grob using:

(Y-offset . ,side-position-interface::y-aligned-side)

can theoretically be cross-staff and needs to have:

(cross-staff . ,ly:side-position-interface::calc-cross-staff)

or a variant.
