This now (as issue 3479 has been committed) outputs
{ R1-\tweak outside-staff-priority #40 -\tweak outside-staff-padding #0  
-\markup \fermata }
As this is a reasonably descriptive, accurate and most importantly  
rereadable output, I would consider the issue fixed, barring protests in  
the next few days.
