You could argue then that \with-dimensions is a placeholder for the sake of  
markup.  Conecptually, I don't see a reason to make \with-dimensions and  
\pad-around play nicely with skylines and not \transparent.

If you change the example slightly, it seems more plausible :

Here, I think the expectation of any user would be that all the foos and  
bars are placed the same in both examples.  There is no way to specify to  
LilyPond that a part of a markup should be transparent but that said markup  
should still could in skylines.

I still think that stencils should carry their own skylines when needed, as  
it would allow for snug inner-stencil placement (very much like the snug  
inner-grob placement we have now).
