Not completely fixed yet.

1) its new more-uniform lower-limit to note spacing allows augmentation  
dots and accidentals to be closer to a neighbor note than their parent; we  
can increase between-note padding to be at least as large as  
accidental-to-parent padding to solve this.  This might also sway the  
line-breaking decision mentioned in "dixit dominus" above.

2) In comments 37-40 Karol and Janek preferred the unequal spacing if the  
inserted column is for a rehearsal mark, in some cases, so they will want  
to adjust the spacing for this kind of inserted columns.
