The CallCentric account and web portal does allow you to set up both call
treatments and forwarding of calls to another place; not just to a phone
number, but you can also enter a SIP address as the destination. And you
can filter it for incoming calls from a specific number(s), during specific
times, if you are busy on another call, etc.

So you could set up a linphone SIP account, then go to the CallCentric web
portal and set up whatever specific forwarding you wanted to that linphone
SIP address, then the calls incoming to CallCentric would get forwarded to
your linphone account, and should get a push notification to the app.
