Sorry, for all other OS but I don't know solutions for MAC OS.
But perhaps some hints which can help setting up the system with
the current screenreader:
1. Set the Option -show_cursor in commandline or lynx.cfg.
2. In the optionmenue (type 'o' when lynx is started)
set the keypadmode 'K' to links_and_forms_are_numbered.
I hope, this will help the screenreader to follow the screen better.
As I know, you can configure outspoken to special events,
which are detected by colour attributes.
You can use the color-definitions in .lynx.rc to define very
precisely such events. For example the error messages may be
tailored in a certain color and Outspoken will watch the occurence
of this events. Additionally it might be helpful to increase the
time for the appearance of info-messages, Warnings etc.
on the screen. They may be configured in .lynx.rc. Search for
INFOSECS.
