In direct email with Adam, we figured out the problem. I'm pretty sure he was using Lynxlet.app. I'll describe the issue here for the archives.
One problem for Lynxlet.app is that it is fairly easy to accidentally reset Terminal's defaults to use the Lynxlet settings for new windows. If one selects File>User-Settings-As-Defaults when a Lynxlet window is in front, Terminal will use lynx instead of a shell for all future windows.
In 10.4, there is no easy way to fix this from the UI. The simplest solution is to delete Terminal's preferences file:
~/Library/Preferences/com.apple.Terminal.plistIn 10.5, Terminal's Preference window can be used to reset the default window setting.
