Like this?:

    +  ("``" "̀") ; U+0300 COMBINING GRAVE ACCENT
    +  ("''" "́") ; U+0301 COMBINING ACUTE ACCENT
    +  ("a''" "á") ; U+00E1 LATIN SMALL LETTER A WITH ACUTE
    +  ("i''" "í") ; U+00ED LATIN SMALL LETTER I WITH ACUTE
    +  ("u''" "ú") ; U+00FA LATIN SMALL LETTER U WITH ACUTE
+  ("uu''" "ṹ") ; U+1E79 LATIN SMALL LETTER U WITH TILDE AND ACUTE

    +  ("e``" "è") ; U+00E8 LATIN SMALL LETTER E WITH GRAVE

This looks good.

Looking at this, I am beginning to think that Dominik Wujastyk’s idea to
put that into a separate file sa-iast-vedic.mim is probably better.

Because one might want to use ' as apostrophes or single quotes and one
might also want to use ` to quote something.

I agree. Since we already got this far, it would be good to have this sa-iast-vedic file - once upon a time I used this more frequently; and it is not unlikely that others would too (given that vedic accents are pervasive; and vedas + sanskrit grammar are a perennial topic of interest).
