I just upgraded nmh on Debian and the default replcomps file doesn't use
the old "In-Reply-To:" field anymore. It prepends "Comments:" as in:
Comments: In-reply-to Ken Hornstein <address@hidden>
message dated "Thu, 01 Jul 2004 15:39:25 -0400."
The relevant changelog entry is probably:

except that the bug #3356 from Satyaki contained a patch that did not
match this new bahaviour:

Also, I think adding "Fcc: +outbox" by default oversteps the user's
space and should not be done.
