Follow-up Comment #22:

0 should work fine there, but yes, nullptr is better.

I don't know how to test whether the user will want to use ftell/fseek on the
stream.  I'm also not sure whether the test is needed if the file is only
opened for writing.  It seems better to me to try to get the right result.  If
someone is worried about performance, maybe don't open the file in text mode.
