MEANS

I've just pushed a re-implementation of MEANS.  This is a command which
at a first glance seems rather simple but in fact is suprisingly
tricky.   However it has been made a lot easier by Ben's pivot tables.
Thanks Ben! 

Anyway I've tested it quite thorougly and tries to comment the code as
best I can.  But I'd appreciate if anyone has the time to look over
the code and make feedback that seems pertinent. 

Basically I'd like to have feedback which will make it easier for
other people to understand the code, and for me to understand it in
three years' time.
