I recommend looking at "a2x" (by Ralph Swick) as an example of a
general interface to all of the test extensions -- because it was
written to support doing "anything" from a word-based speech
recognizer.  See http://www.cl.cam.ac.uk/a2x-voice/a2x-faq.html
for a starting (though possibly out of date) set of pointers.   It
doesn't look like it solves your modifier-key-state problem,
though...
