I have used spamass-milter to handle piping incoming mail to
SpamAssassin. Which has worked fine for the past year until I needed to
tag mail with the [SPAM] subject line which requires the ability to
rewrite messages (no -m in spamass-milter) Without it it has issues:

It's called up by:
spamass_milter_flags="-f -p ${spamass_milter_socket} -r 10 -u spamd"
and in main.cf:
# Milters for mail that arrives via the smtpd(8) server.
# See below for socket address syntax.
milter_connect_macros = b i j _ {daemon_name} {if_name} {if_addr}
milter_end_of_data_macros = b i j _ {daemon_name} {if_name} {if_addr}
{mail_addr}
smtpd_milters = unix:/var/run/clamav/clmilter.sock,
unix:/var/run/spamass-milter.sock
Adding back in -m to the spamass-milter command line removes the error,
but it doesn't have the ability then to rewrite subject lines. :(
