Axiom sources contain two copies of 'util.ht': one is
'src/hyper/pages/util.ht', the second is
'src/share/doc/hypertex/pages/util.ht'.  This causes build problems
since when re-making different copy may be installed. Also
'ht.db' is build when pages from 'src/hyper/pages' (including 'util.ht')
are installed. If 'src/share/doc/hypertex/pages/util.ht' is intalled
later it makes 'ht.db' out of date.

I would propose removing one copy of 'util.ht' (removing
'src/share/doc/hypertex/pages/util.ht' would make Makefile logic
simpler, so IMHO this one is preferred).  Also, 'src/hyper/pages/ht.db'
us useless (it should be re-created by the build process) and
potentially confusing (it may easily get out of date), so
it should be also removed from the repository.
