So it seems we go from Unasked to either None (meaning as far as could be determined with patient co-operation) or any combination among +- Specific allergies +- Something the patient cannot clearly-enough recall (but we should put in what we can) +- Something the patient prefers to withhold * (but if patient acknowledges withholding e.g. for personal reasons we should put in a comment to this effect) * we should consider that the patient *could* be willing to share the information at a future visit, especially depending on the nature of future complaints or of Rx we would offer qed ?
