Greek Phi is great for no allergies. ? is great for unasked Maybe "!?" would work for "asked but no data obtained" ? The tooltip would reveal the long form.In state = 1 the triggers would be short-listed. Again, the tooltip revealsmore details.
The above symbolism (and overall scenario support) sounds fine. It covers more cases than some people (like Richard) may feel that they require, but the only cost to people who would rather it be kept simple is:
1. when a patient would answer (on routine inquiry) that they had no allergies, it would require an extra step for the clinician to clear the "?".
(I do think the dialog box could be improved but will leave that to the post-Japan "other" requests)
2. #1 may be acceptable (to those who may rather do without) because it would need to be done only once per patient and we could maybe agree that the greatest risk of failure of capture would be at the point where a patient with allergies joins a praxis. Subsequently, there is a decent chance that any new allergies would be encountered in the course of care or, if not, some correspondence about new allergies may have been received.
3. once the "?" flag is cleared, the fact that people may ask anyway at each new prescription is not opposed by the absence of the "?"
4. any entries apart from the "state" are optional :-)
