I have never used refer to its fullest capabilities. Luckily I have only
ever had only tens of references in my papers, I have just numbered them
manually and used a '.RL' to print them. I need to do this properly now
because I have to worry about 100+ references in the current report.
If I use footnote, and I do not want to use them for bibliographies, I simply
insert the string \*F in the test, put my test between FS/FE and it all just
happens, as in
Here is a footnote \*F and it will appear at the bottom
.FS
Some footnote text.
.FE
of the pages.
However, by using a small extension to the MM macros suggested by
Andy Dougherty, the following manual numbering stuff works because
I do not sort the references. I use a '.RL' for the display of the
accumulated references when they are flushed.

join-authors " & " ", " ", & "

Here is the first reference [1] about strength of composites.

There has to be a better way to ensure that the numbering is automatic
and I do not have to keep track of it.
Any ideas?
