It's a good question.

The usual way to handle this in the main text is to use @deffnx and the
like.  Thus:

@deffn plus ...
@deffnx + ...
@deffnx addition ...

Then index entries will automatically be made.

If you don't want them in the main text, then the only recourse is to
explicitly put in @findex (or whatever) commands at the @deffn plus, as
Eli suggested.  Unfortunately Texinfo does not currently support "see"
or "see also" index entries, which is what would be ideal.  

Does that help?  I'm happy Texinfo is being used for something as big as
Maxima!
