LGTM I'm not sure the "programmed smart" is a good idea. The rules are rather obscure, without full consensus, and even if fully implemented may well not do what the engraver wished in all circumstances. I think a better approach might be to let the engraver place these annotations by hand if the simple default positioning is not to his taste. Better tools for manually controlling the positioning might be desirable, though.
