Hello,
At first sight I would change one line of your .profile like this to include
the lilypond binary in your path:
export PATH=/opt/local/bin:/opt/local/sbin:$PATH
to
export
PATH=/opt/local/bin:/opt/local/sbin:/Applications/LilyPond.app/Contents/Resources/bin:$PATH
