[Nmh-workers] mh-format(5) terminfo support

I've added a few new instruction to mh-format(5) that can use some basic
terminfo(5) capabilities, like bold, standout, underline, and foreground
and background color changing.  The man page for mh-format(5) has been updated
and there's an example scan.curses in the etc directory.  If anyone has
any questions about this, don't hesitate to ask.
