Good investigation.  Yeah, hypothesis 3 is out.

As to the vmware driver being the source, I'd have to guess, maybe, but
probably no.  I was imagining you had some special driver for your card and it
might have a bug, but that would have to be for some fly-by-night driver,
wouldn't you think?  I mean, if vmware is a popular driver and gets exposure
to lots of different cards and so on, such an obvious mistake as the window
coordinate orientation would certainly get immediate attention.

Plus, don't we want people to use 3D acceleration if it is available?  Is 3D
acceleration using hardware features?  Or is it just another type of software
library extension?
