Indeed, the <p> could be omitted. In fact it is omitted in docbook and
xml output to match with what makeinfo does. In the past it wasn't easy
to do context-sensitive formatting of the paragraphs, now with
command_stack_at_begin it is easy.
I put it in the TODO file, but I'd rather finish what I am currently
doing with indices before that change (and the change in spaces
after/before @item) that will need to sync many tests.
