empty index entries

    I have done that.  Should it be restricted to real index entry commands
    like @cindex or also to to index entries from @def* commands and 
    @item in @(v|h)table?

I think it should be anywhere, however it is generated.  An index entry
should never be empty, as far as I can imagine.
