Ddrescue stops instantly if the drive disappears from /dev. If it simply
returns a read error, ddrescue can't know if the drive has failed fatally.
Try this and, if needed, increment the argument to -e after each reboot:
ddrescue -e0 -RT /dev/[source] /dev/[target] rescue.log
