Details:
The VARIABLE WIDTH command appears to have previously assert-failed in a
reliable fashion; I don't see how it could have worked. Also, there were no
tests that exercised it.
This patch fixes the problem and adds a simple test for VARIABLE WIDTH,
VARIABLE ALIGNMENT, and VARIABLE LEVEL.
