RE: Headers not being added

The only time I've ever had headers not being added is when the milter is
somehow unable to communicate with spamd (it seems that in my case this is
usually a result of spamc not being able to talk to spamd on a remote
machine).  I think that's a little different scenario than the one described
earlier, though.
