Why couldn't you exceed the maximum only with bonuses given by items?
This seems good as well. Although I have nothing against increasing
both, which is just the same actually.

The problem only occurs when you modify direct values like health or
mana. IMO items should never change these values (except for special
cases) but rather change *modifiers* like resistance, and so on. That
way you can't run into the problems you described above.
