If it happens again, why not first read back the existing bootloader
code, and compare it against the original code?

Also, maybe it's some kind of bug in that bootloader that makes it not
work, depending on the existing application ROM pattern?  Sounds
weird, but it would also explain why it starts working again after
erasing the chip.  If your attempt from step#1 above saves the entire
image somewhere, you should be able to verify the reproducability of
it, too (save entire image, chip erase, reprogram entire image => does
it happen again?).
