completion-ignore-case: documentation bug

Please describe exactly what actions triggered the bug
and the precise symptoms of the bug:

If I recall correctly, setting completion-ignore-case to true used to
result in case being ignored when completing reads of filenames.  Over
the past few months, emacs's indifference to case in this context tended
to fade inexplicably after a random amount of time, and with my latest
build, it fails altogether, even right after emacs is started.  I found
out about read-file-name-completion-ignore-case by reading the NEWS
file, but it would be good to mention it in the documentation for
completion-ignore-case.  Even better would be to warn people of the
change on startup, if at the end of initialization
completion-ignore-case is set to t and
read-file-name-completion-ignore-case isn't.
