I'm actually in similar situation and what I actually do is I have a special 
gateway connection defined in ~/.ssh/config like so:

  Host foogateway
      BatchMode yes
      HostName real_gateway_hostname
      User foo
      LocalForward X H:P
      ...

Then I just start that connection with 'ssh -fN foogateway' in the background.  
This actually has an advantage that it is setup once and keeps running until 
you shut it down; consider that doing it in Fabric in setup/teardown would 
actually do it for each fab invocation.
