You may have incorrect metadata in your revlib.  Incorrect file 
permissions on library revisions aren't currently detected by the 
corruption-detection routines.  I had some spooky permissions problems a 
while back, and that turned out to be the cause.

What you can do is,
1 apply the changes
2 undo the changes
3 delete the permission changes from the undo changeset
4 redo the changes.

Did I say it was pretty?  Sorry, I lied.
