I was only wondering (seeing especially your latest PDF) how to resolve the 
conundrum where we might desire to print more information than will fit in a 
single row on an A4 page. It seemed to me that to position the "notes" 
information on the line *below* the main medication, indented, would

a) help to maximally use the top row-per-medication for information *other* 
than the notes
b) offers efficiency, in that not all will have notes, and it may be possible 
in such cases to conserve a row

I only don't know whether a tabular format can allow there to be any 
positioning ("wrapping") of information onto a second line. Do medications 
require a grid in which the columns display the type of entity or would 
medication lists be sufficiently self-explanatory? If we went with a non-table 
view, then the {episode information, intended treatment aim} could be appended, 
despite that this approach would not be as visually effective where the 
organization of the medications was by episode.
