I think I found the answer for "informed consent".  In the 2002 ethics
manual, there is a chapter called "informed consent in research".
This chapter contains a list of questions which outline how to ask for
informed consent.

I revised your informed consent section, following the ethics manual.
If you logout and go to the "create a new account" screen then you can
see what I came up with.

This should cover the DSM biographical information and any other data
we collect.
