FLAGS DCP just sets the 'digital copy permitted' flag
INDEX 00 00:04:00 marks the beginning of the track pre-gap...
VCDGEAR is obviously broken, as it doesn't understand the complete
CUE-syntax!
btw, there can be also INDEX 02, 03... 99 marks, which would define
sub-indexes (they are created when you add entry points)...
previous vcdimager versions created only INDEX 01 lines... you can
safely remove the new lines...
