Actually, it's exactly the same thing.  The ambitus engraver catches every
note in the score, and adjusts the range as needed.  Then, when it's
printing time (after the whole score has been read through), it prints the
appropriate notes.

The score is not set one note at a time.  During the "Preprocessing
graphical objects" phase the entire score is scanned.  Then the
page-breaking and line-breaking are done, and then the output is sent to the
file.
