I've been wanting to generate videos from lilypond, and so far it seems like the best approach (scrolling, rather than paginated) is:

It does not allow anything but 7-bit characters in the entire score, does not understand modern lilypond syntax (e.g. "c2 2" ) and has a number of other serious bugs.
