Follow-up Comment #21:

Closing report.  The problem lies outside of Octave in Makeinfo.  

Summary: it seems necessary to use either an old version of Makeinfo or the
very latest version of Makeinfo, but to avoid intermediate versions.  This is
important for anyone who will be packaging Octave into a binary install --
They need to include an up-to-date version of Makeinfo in the bundle.
