I think this will fix you're problem


The line:
  $sth_select_all->execute($HEADERS)
   or die "$err_exec_sql $dbh->errstr";

should be:
  $sth_select_all->execute($HEADERS{phpid})
    or die "$err_exec_sql $dbh->errstr";
