Concerning a) and c) see issue 1787.  Maybe that issue should be fixed in a  
different way that was done; fortunately the guy who wrote that workaround  
actually included a comment.

b) yeah, stems are rounded rectangles with a fixed amount of roundedness,  
and stafflines are lines which end in half-circle, thus the roundedness  
depends on thickness of the line.
