What do other applications do when you click outside a dialog?

Nothing.  The click is simply not passed to the application by most
toolkits.

Is there a "standard" idea of what ought to happen?

I don't know of any application except Emacs 21.3 and erlier that does
anything.  I think doing nothing is what users expect.

We should probably try to get ESC to close the dialog into the Lucid
toolkit.  That is consistent with Motif/Lesstif, MS Windows, MacOS and
GTK.
