Looking at the current GUI, I might suggest

- limiting a visual indication per item to the EMR tree view
- omitting any visual indication within the Notes 

In the EMR tree view, I might suggest

- appending the visual indication (when present) after the Health Issue or 
Encounter

- the visual indication could be parens ( ) or brackets [ ] e.g.
        parens, within which is the coding system from a code was assigned
                e.g. (ICD9)
        parens, within which is the assigned code
                e.g. (401)
        parens, within which is the pairing that was used
                e.g. (ICD9: 401) <-- note insertion of space for clarity
        parens , within which is a symbol
                e.g. asterisk or bullet or checkmark
                e.g. (*) or (√) or (•) 

- the actual paired value(s) could be shown in the right pane, for example for 
Kirk under his EMR statistics below, any codes that had been applied could be 
inserted between the text description and the status of the problem:
